[Summit] Overnight parking Opt-Out petition form

I'm actually pretty okay with the new policy. I do wish it was possible to
call in overnight guests for free, but that's about my only gripe with it.

At least now everyone has an option, even it if costs money. Previously, a
lot of people were completely without parking options. I know this list
leans heavily anti-renter, but they are a vital part of this (and most)
neighborhood(s).
